DOT Exam includes:
- Commercial Driver’s License
- Medical Certificate
All providers at Workmed Midwest are FMCSA Certified Medical Examiners.
On commercial drivers’ exams You will need an A1C in the last 3 months if diabetic, copy of compliance report if on a c-pap, a copy of stress test if you have heart stents a prior heart attack, angina, or bypass.
Also, if taking medications bring a medication list. MCSA-5895 can be completed by your health care provider. Some medications require input from your personal physician.MCSA-5870 for insulin use and MCSA-5871 vision must be completed prior to exam and within 45 days.
MCSA-5875 Is voluntary for non-insulin diabetes with medication is recommended to be completed by your healthcare provider to expedite your visit.

Fitness for Duty exams are a unique opportunity for employers to use preventive medicine to objectively access employees in relation to their specific job duties. These exams ensure employees are physically able to perform the assigned scope of work and are not a hazard to themselves or others. Fitness programs are essential to industries that have intense physical demands such as Law enforcement, Firefighters, transportation, manufacturing, or construction. Evaluations can prevent impending threats to employee safety, reduce the possibility of injury, and lessen the potential for liability.

Employment exams are different than the full physical you have with your regular physician. The exam is directed to the essential parts of the job. Bring gym shorts and comfortable top. You may be required to demonstrate lifting, climbing, or other activities. You likely will have some parts of the exam which are more familiar to a regular physical exam such as the doctor listening to your heart and lungs.

DRUG & ALCOHOL TESTING
All collectors at Workmed Midwest are trained and certified DOT collectors. We are an appointment-based clinic. Drug tests can be scheduled from 7:30 a.m. to 4:00 p.m. M-TH, 8:00 a.m. to 2:30 p.m. F. Please note, any drug test that would require Workmed to remain open longer than our normal clinic hours will incur extra charges.
REASONABLE SUSPICION SUPERVISOR TRAINING:
Employees under the influence of drugs or alcohol at work threaten the safety of themselves and everyone else working around them. Reasonable suspicion training develops a common understand of how to take action without accusation or discrimination whenever an employee shows signs or symptoms of being unfit for duty. Most often, the frontline supervisor is the first to identify a potential concern. However, without the proper training on how to respond to these delicate situations, supervisors may make incorrect choices that have the very real potential for costly legal consequences.
Training is available for both DOT and NDOT supervisors. Federal Motor Carrier Safety Administration requires anyone who supervises DOT employees MUST go through DOT Reasonable Suspicion 2 Hour Training!
Training is available periodically throughout the year via classes held by Workmed Midwest or if you would prefer, we can also do the training at your facility!
