What to expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to deliver the best care.
If you're coming in for a pre-placement physical, DOT exam or OSHA required exam we suggest you bring the following; gym shorts, t-shirt and comfortable shoes. An occupational medical exam has some similar components of a regular physical exam, but may require lifting, bending, climbing, or other physical activity. Also, bring a list of all medications you are currently taking.
On commercial drivers exams you will need an A1C in the last 3 months if diabetic, copy of compliance report if on c-pap, a copy of stress test if you have heart stents a prior heart attack, angina, or bypass .Bring medication list.
Injury care: bring any reports, x-rays,exam notes, or care notes rendered prior to the visit.
